Q. What insurances do you accept?
A. Dr. Antonoff is a preferred provider with the following (in no particular order): Premera, Regence, Aetna, Cigna (includes Healthways), First Choice (includes Moda), Lifewise, Humana, and Medicare. Some insurance companies including Regence and Cigna may require PRE-AUTHORIZATION for any chiropractic visits. Please check with your insurance provider ahead of time! We also offer a cash discount for paying at the time of service if you choose not to bill insurance or simply prefer to pay cash.
Q. What payment methods do you accept?
A. We accept cash, check, Visa, MasterCard, and American Express.
Q. I have both primary and secondary insurance coverage. Am I responsible for billing my secondary insurance?
A. Our clinic does not bill secondary insurances. Contact your secondary insurance company for instructions on submitting your claims for reimbursement.
Q. I recently changed insurances or just got insurance that would have covered a few past visits. Is it too late to bill my insurance for those visits since they have passed already?
A. No. As long as we receive that information and send the claim within the insurance company's stated timeframe, we are generally able to bill for visits in the recent past.
Q. When I get a statement from my insurance company in the mail, do I need to bring in a copy for you?
A. No. When the insurance company sends you an Explanation of Benefits (EOB), we receive our own copy as well. Thank you for thinking of us though.
Q. My company offers us Health Spending Accounts/Flex Spending Accounts (HSA/FSA) for which we have to provide itemized receipts. Can you produce those?
A. Absolutely! All you need to do is ask someone at the front desk and we can print off the necessary documents for you. We can provide receipts and ledgers when necessary; we just ask for 24 hours' notice.
